Business Credit Application

The above information is requested for the purpose of evaluating and extending credit to your company. Unless otherwise stated our standard terms are Net 30 Days. COS maintains a lien on all purchases until paid for in full. Applicant agrees that, in the event of collection and/or repossession (at COS's option), applicant will be liable for all costs related to the collection, repossession and/or costs of reselling, including, but not limited to collection agencies, court costs, refurbishing, repairing, reclaiming and transporting. Applicant authorizes COS to contact above references, or such other sources as it may deem appropriate, and further authorizes the release of such information and to hold harmless COS for pursuing such information and for such decisions as may be made as the result of such investigation. Customer further agrees to hold harmless any of the above references, or other agency releasing information about customer once they have been presented with this release.

COS reserves the right to assess a monthly service charge equivalent to 1.5% on all accounts not paid within stated terms or other such fees as it may deem appropriate and customer agrees to pay same.
